Your Clinic Visit

Summit Cardiology welcomes you as a new patient to our practice. Please arrive 10 to 15 minutes early for your first appointment.

Please remember to bring the following to your first visit:

• Completed registration form and papers
• A list of current medications, dosages and times taken
• Complete health history
• Comfortable clothing and walking shoes if you are scheduled for a stress test
• Any pertinent medical records from your referring physician such as EKG or lab tests
• Insurance card and information about your coverage
• Primary care physician authorization, if required by your insurance
• Office co-payment, if required by your insurance plan

Please follow the directions below to find out office:

From the North (Southbound I-5)

• Travel South on I-5 to the Northgate Way Exit (Exit #173)
• Take the Northgate Way westbound ramp
• Turn right onto N. Northgate Way
• Turn right onto Meridian Avenue North
• Turn LEFT onto N. 115th Street
• Turn right to enter the Northwest Hospital & Medical Center campus
• Follow signs to the McMurray Medical Building

From the South (Northbound I-5)

• Travel North on I-5 to the Northgate Way/1st Ave Exit (Ext #173)
• Turn left onto 1st Avenue NE
• Turn left onto N Northgate Way
• Turn right onto Meridian Avenue North
• Turn left onto N 115th Street
• Turn right to enter the Northwest Hospital & Medical Center campus
• Follow signs to the McMurray Medical Building
